The Outreach Services Coordinator will serve as point of contact for the Rockdale location between community members. This position will spearhead community outreach efforts to educate community and key partners of available services, resources, and conduct health awareness activities. The position will support multiple grants and enabling services through planning, monitoring, data collection, statistical analysis, and enrollment assistance.


• Plan, prepare and implement projects and procedures for effective community outreach and enrichment.
• Assess clients' involvement in services, identify barrier to care and readiness to access care, and work with clients to eliminate barrier where applicable.
• Identify and engage with key community leaders and relevant organizations whose work is aligned with the vision and mission of Oakhurst Medical Center.
• Conduct public education activities to raise awareness about coverage options available under Medicaid, Children's Health Insurance Program (CHIP), and the Marketplace.
• Coordinate and collaborate with other Community Health Centers, Primary care Association, and local resources to provide belter assistance to potential members.
• Provide assistance in determining eligibility for coverage options and facilitating enrollment in affordable health plans.
• Locate upcoming events at which to conduct outreach activities, and maintain calendar of such events.
• Work directly with front office, billing, and clinical staff to assure patients are assessed for health insurance eligibility to include sliding fee scale where applicable.
• Assist patients or other legally appropriate parties, in a customer focused manner, with completing and understating the registration process, required documentation and insurance information.
• Serve as an essential link between patients and all other care providers through referrals.
• Responsible for documentation of program data and statistics.
• Prepare reports and documents as needed or requested.
• Obtain and maintain required certifications, licensure, or appropriate training to perform job responsibilities.
• Provide intake and assessment of patients identified through Rockdale Medical Center.
• Conduct patient education/health education.
• Work with hospital to coordinate care at a medical home, including follow up on appointment adherence.
• Provide referrals and support for social support services.
• Help coordinate transportation for patients to appointments.


  • Bachelor's degree in public health, social services or related field preferred. 
  • Minimum of 2 years experience in a successful community outreach/recruitment and volunteer coordination.
  •  Valid Georgia driver license required.
  •  Excellent phone etiquette.Excellent communication (te lephone, email or in person), organizational and customer service skills.
  • Must be competent in Microsoft software applications.
  • Ability to connect with others and forge strong relationships.
  • Ability to relate to individuals and families of varied cultural backgrounds and economic circumstances.
  • Ability to communicate effectively in a team environment.
  • Ability to speak effectively in front of large audiences
