Overview

Performs the functions of a Registered Nurse in assessing, planning, implementing and evaluating in the clinical area under the supervision of a Licensed Independent Practitioner (LIP).  Adheres to the standards related to the registered nurses responsibility to apply the nursing process (adapted from the American Nurses’ Association Code for Nurses and Standards of Practice).  Plans, supervises and coordinates the orientation, training, and daily activities of the License Practical Nurse (LPN) and Medical Assistant (MA). This position is also responsible for coordinating and tracking the delivery of care for CMS members and other OMC patient population parameters necessary to ensure that the highest standard of medical compliance is obtained within the organization.

Responsibilities

All duties to be performed within the legal boundaries of nursing practice based upon knowledge of statutes and regulations governing nursing, and in compliance with all company policies and procedures while maintaining company standards:

 

  • Implements strategies, coordinates, monitors, and evaluates options and services to provide for patient/client participation in health promotion, maintenance and restoration, while ensuring the appropriate use of OMC clinical resources.
  • Serves as the Nurse Manager and patient intake coordinator or patient intake nurse for the facilities. Performs and directs nursing care and patient education as appropriate; develops and monitors patient care plans for assigned patients, which includes goals and prioritized nursing approaches or measures derived from the nursing diagnosis; completes treatments or procedures ordered by physicians or mid-level providers and assists them with any procedures when necessary. 
  • Expedites and coordinates appointments for assigned Oakhurst patients to ensure client is engaged in achieving positive health outcomes.
  • Directs patient flow in the medical treatment area and assures the accuracy and completion of medical records. Maintains an adequate stock of medical supplies and assures the equipment is in good repair and working order.  Assures that all clinical record logs and reports are accurately and thoroughly prepared, and that they are submitted timely.     
  • Manages scheduling of the Medical Assistants as needed for efficient operation of the center, determine that the care and/or activity to be performed would be based upon orders or directions of a licensed physician, licensed dentist, licensed podiatrist or person licensed to practice nursing as a registered professional nurse.
  • Manages, facilitates and evaluates the ongoing training for the Medical Assistance and LPNs, to unsure that they remain competent in the knowledge and skills necessary to perform their duties.
  • Adheres to the standards related to the registered nurse’s responsibilities in assignment of patient activities to unlicensed assistive personnel (UAP).
  • Continues to maintain appropriate license and certifications, seeks educational resources and creates learning experiences to enhance and maintain current knowledge and skills appropriate to her/his area of practice.
  • Knowledge and ability to recognize urgent or emergent patient care situations, seeks assistance of the MD/DO or mid-level providers, and initiates appropriate emergency interventions as directed.
  • Communicates, collaborates and functions with other members of the health team to provide comprehensive case management.
  • Participates in off-site activities which include meetings, seminars, workshops, health fairs, and community presentations.
  • Assists in maintaining Occupational Safety and Health Administration (OSHA) and Clinical Laboratory Improvement Amendments (CLIA) regulation for OCHC.
  • Makes referrals to outside agencies upon request of the physician.
  • Maintains confidentiality of electronic, written, and/or verbal patient/employee information in accordance with the policies of the Medical Center.
  • Assist in ensuring Healthcare Effectiveness Data and Information Set (HEDIS) related preventive screenings, tests and vaccines are performed timely and in an appropriate manner with Oakhurst existing providers.
  • Responsible for the extraction of data of eight domains of care: effectiveness of care, access/availability of care, satisfaction with experience of care, health plan stability, use of services, cost of care, informed health care choices, and health plan descriptive information.
  • Planning, managing, case assignment and of the immunization outreach project by conducting community outreach and education for children, their parents and community members.
  • Participate on a clinical care team that is focused on achieving HEDIS performance measures as outlined by National Committee of Quality Assurance (NCQA) with an emphasis on the certification of Patient-Center Medical Home (PCMH) model.
  • Develop, coordinate and monitor the pregnancy centering of patients for the organization.
  • Serve on the quality improvement committee.
  • Develops the clinical data management plan and maintains it throughout the lifecycle of the study project and ensures it is followed according to the study design and requirements.
  • Assist in defining and/or the creation of data listings, summary tables, data specifications and/or processes in preparation for statistical review and/or data management audits.
  • Coordinate and communicate with Medical Director and Director of Nursing on a consistent basis to address clinical team requests of related data management project plans.
  • Develop forms for receiving, processing or tracking of clinical data.
  • Assist in the maintenance of electronic health records and patient management systems and databases. Helps submit encounter data, including meaningful use, patient center medical home and third party payers.
  • Develops and generate reports that present enrollment and patient health information for staff and annual health care plan use.
  • Responsible for implementing and maintaining accurate input of the Uniform Data Collections Systems (UDCS) data into a compliance system used to maintain patient information.

Coordinate collection of data for the annual Uniform Data System (UDS) report.

Qualifications

Education/Experience/Skills

  • Bachelor’s degree in nursing and/or higher level of educational program in the health care administration, nursing, or adult education required.
  • Current, unrestrictive license as a Registered Nurse in the state of Georgia.
  • At least three (3) years’ experience in an ambulatory health care setting as well as possess expertise in management of clinical staff.
  • At least two (2) years of experience in case management care coordination in an HMO/CMO environment; or any combination of education and experience that would provide an equivalent background.
  • Background in working with adult, pediatric and multi-cultural populations.
  • Current Healthcare Provider Cardiopulmonary Resuscitation (CPR) course completion card.
  • Current Basic Life Support (BLS) certification for healthcare providers.
  • Must be competent in Microsoft software applications.
  • Excellent communication, organizational and customer service skills.
  • Excellent phone etiquette.

Language Ability

  • Ability to communicate effectively in a team environment.
  • Ability to speak effectively in front of large audiences.
