  • Overview

    Performs the functions of a Licensed Practical Nurse in assessing, planning, implementing, and evaluating in the clinical area.  Calls for high level of independent judgment.  Ability to set patient care as a priority.  Assists in the training and orientation of new personnel.    

    Responsibilities

    • Monitor and handle prescription refill requests in compliance with OMC policy and clinical guidelines in a timely manner on a daily basis.
    • Advocate for the patients to obtain timely prescription refills.
    • Collaborate with the quality team to assist with compliance to medication adherence guidelines.
    • Monitor and respond to portal messages requesting clinical advice in collaboration with the providers in a timely manner.
    • Provide advice that respects the culturally diverse individuals we serve and reserve caller’s autonomy, dignity and rights.
    • Effectively assess patients based on the patient’s own evaluation and responses to questions.
    • Schedule follow up appointments as needed to facilitate the prescription refill.
    • Utilize the electronic medical records to access patient records to facilitate a timely response to the patients need for clinical advice and prescription refills.
    • Document all communication with the patient in their medical record.
    • Participate in team huddles as necessary to promote communication of patient needs.
    • Use excellent customer service to deal diplomatically and constructively with callers.
    • Effectively prioritize, coordinate, organize and handle several tasks at once in a fast-paced environment.
    • Maintain appropriate nursing certification through clinical medical education or continuing educational units.
    • Participate in departmental meetings.
    • Participate in any pertinent in-service education planned by the center to improve job performance.
    • Participate in off-site activities which include meetings, seminars, workshops, health fairs, and community presentations.

    Qualifications

    Education/Experience/Skills

    • Graduate of an accredited school of nursing.
    • Currently licensed as an LPN in the state of Georgia.
    • Current AHA Healthcare Provider training (BCLS).
    • Current CPR certification.
    • Thorough knowledge of nursing procedures in an ambulatory health care setting and clinical expertise.
    • Background in working with adult, pediatric and multi cultural populations.
    • Familiarity in case management with obesity, hypertension and diabetes.
    • Familiarity and basic knowledge of Electronic Health Records (EMR).
    • Excellent communication, organizational and customer service skills.
    • Excellent phone etiquette.

    Language Ability

    • Ability to communicate effectively in a team environment.
    • Ability to speak effectively in front of large audiences.

    Math Ability

    • Basic mathematical skills.

    Reasoning Ability

    • Ability to multi-task and prioritize.
